Hittite Microwave is committed to following the export laws of the United States. Certain customer, end-use information and U.S. export licenses may be required prior to fulfilling specific customer requests.

U.S. based exporter and international purchase orders will require Hittite’s Export Compliance End User Certification Form to be completed prior to Hittite accepting and processing the purchase order. Please fill out this editable pdf form on-line, and then print and sign it. Scan and Email or Fax the form to the Hittite Sales Administrator assigned to handle your order.

If requested, please complete and return this certification promptly. All fields are required, and Hittite may request additional information if further clarification is needed.
